We found during troubleshooting that the School CEEB must be mapped as a system field (not a form field) for the registration block to function correctly. When it was mapped as a form field only, the form was only creating a registration for the first person entered. - GQUEEN 07/16/2025Please choose a school from the drop-down list, if possible.District NameGrades Serving *Grades Serving *Elementary06070809101112Is your school designated a Title 1 school? *Is your school designated a Title 1 school? *YesNoPercentage of students that qualify for free or reduced lunch?Percentage of students that are part of an ethnic minority?Percentage of students that would be first-generation college-bound students?Teachers & Admin Overseeing MESADeleteContact Person _ID_Be sure to provide contact information for both the Teacher(s) and the Principal/Admin(s) that are overseeing MESA at your school.First NameLast NameTitleEmail AddressRoleMESA AdvisorPrincipal/AdminAdd another Contact PersonMESA Program InformationSelect your program fee option: *Select your program fee option: *Full ProgramA La CarteOur school agrees to the Memorandum of Understanding and agrees to pay the fees associated with the program options selected above.Teacher Overseeing MESA:Click to Sign...Submit
